Pyeloplasty in DelhiPediatric PUJ Obstruction Treatment
Has a scan shown a swollen kidney in your baby or child? Dr. Pritesh Maheshwari, AIIMS-trained pediatric surgeon and pediatric urologist, assesses hydronephrosis and PUJ obstruction and performs pyeloplasty in Delhi when surgery is genuinely needed.
Finding out that your child’s kidney is swollen, often on a routine pregnancy scan, is worrying. Many children with hydronephrosis never need an operation. Some, however, have a true blockage where the kidney meets the ureter, called pelvi-ureteric junction (PUJ) obstruction, and pyeloplasty is the standard operation to correct it.
Dr. Pritesh Maheshwari is a senior consultant pediatric surgeon and pediatric urologist with surgical experience of more than a decade. He completed his MCh in Pediatric Surgery at AIIMS, New Delhi, as batch topper, and sees families at Maheshwari Multispeciality Clinic in Delhi for pediatric urology consultation. This page explains what to expect, from the first scan review to recovery after pyeloplasty surgery in Delhi.
THE CONDITION
Understanding Pyeloplasty and PUJ Obstruction
What Is PUJ Obstruction?
Kidney→Renal pelvis→PUJ→Ureter→Bladder
The kidney collects urine in a funnel-shaped chamber called the renal pelvis. Urine then drains down a thin tube, the ureter, to the bladder. The point where the two meet is the pelvi-ureteric junction.
In PUJ obstruction, this junction is narrowed or does not drain properly, so urine builds up and the kidney’s collecting system stretches. Most cases are congenital, meaning the child is born with them, and many are first seen on antenatal scans.
Severity varies widely. Some blockages are mild and improve on their own; others slow drainage enough to put kidney function at risk over time. That is why careful follow-up matters more than any single scan.
Hydronephrosis Is Not the Same as PUJ Obstruction
Hydronephrosis describes what the scan sees: a dilated kidney. PUJ obstruction is one possible cause. Others include vesicoureteric reflux (urine flowing back up from the bladder), a blockage lower down the urinary tract, or a temporary widening that settles by itself. Part of the assessment is working out which one your child has.
What Is Pyeloplasty?
Pyeloplasty is a reconstructive operation that removes the narrowed segment at the junction and rejoins the renal pelvis to the ureter with a wide, healthy connection, so urine can drain freely. It is the established surgical treatment for significant PUJ obstruction in children and can be performed as open or laparoscopic surgery.
WHEN TO SEE A SPECIALIST
Signs That Need a Specialist Review
Many babies have no symptoms at all; the only sign is a scan. In older children, PUJ obstruction may show up in other ways. If hydronephrosis was seen around birth, see also our newborn surgery care.
Seek urgent medical care if your child has a high fever with a suspected urine infection, severe pain, repeated vomiting, or seems very unwell.
Hydronephrosis reported on a pregnancy scan or a newborn ultrasound
A kidney that stays swollen, or swells further, on follow-up scans
Episodes of tummy, side or back pain, sometimes with nausea or vomiting
Urinary tract infections, particularly with fever
Occasionally, blood in the urine or a swelling felt in the abdomen
At the first consultation, Dr. Pritesh reviews your child’s history and every scan done so far, including antenatal reports, because the trend over time tells more than one image. Depending on your child’s age and findings, tests may include:
Ultrasound
To measure the dilation and check the kidney tissue
Diuretic renogram
DTPA or EC/MAG3 scan, to show how much each kidney contributes to overall function and how quickly urine drains
Micturating cystourethrogram (MCU)
In selected children, to look for reflux
Kidney function and urine tests
Where needed
Not every child needs every test. Dr. Pritesh explains why each one is advised and how its result will shape the plan.
TREATMENT OPTIONS
PUJ Obstruction Treatment in Delhi
Treatment is matched to how the kidney is behaving, not to the label on the scan report. There are broadly two paths.
PATH 1
Watchful Monitoring
Many babies with mild or stable hydronephrosis are followed with scheduled ultrasounds and, where needed, repeat renograms. Some are advised a preventive antibiotic. A good number improve without surgery, and careful monitoring avoids an operation that was never needed.
PATH 2
When Pyeloplasty Is Recommended
Surgery is considered when the overall picture points to a significant blockage, for example:
Kidney function on the renogram that is reduced or falling on repeat scans
Hydronephrosis that keeps worsening on ultrasound
Pain, vomiting episodes or urinary infections linked to the blockage
Stones or other complications
The decision rests on the complete clinical picture, never on one scan alone. Dr. Pritesh will talk you through the expected benefits, the risks of surgery and the risks of waiting.
THE SURGICAL JOURNEY
Pyeloplasty Surgery in Delhi
If surgery is advised, this is what the journey usually looks like. The hospital, anaesthesia plan and length of stay are confirmed for each child.
1
Pre-operative assessment
Blood tests, an anaesthesia check and a clear explanation of the plan.
2
The operation
Under general anaesthesia, the narrowed segment is removed and the renal pelvis is rejoined to the ureter, a technique known as dismembered (Anderson-Hynes) pyeloplasty.
3
Temporary drainage
In many children a thin internal tube called a DJ stent protects the repair while it heals. If one is used, it is removed later in a short day-care procedure.
4
Hospital stay and discharge
Your child is monitored for comfort, feeding and urine output, and goes home once recovering well.
5
Follow-up
Clinic reviews and scans over the following months confirm that drainage has improved.
Surgery is planned at a hospital with pediatric anaesthesia and post-operative care, and the clinic confirms the hospital when the treatment plan is agreed. This is the same approach Dr. Pritesh follows for PUJ obstruction surgery in Delhi whatever technique is chosen.
Laparoscopic Pyeloplasty in Delhi
Laparoscopic (keyhole) pyeloplasty performs the same repair through a few small incisions, using a camera and fine instruments. Laparoscopic pyeloplasty is among the procedures Dr. Pritesh offers as part of laparoscopic surgery for children, and for suitable children it can mean smaller scars and a comfortable recovery.
Keyhole surgery is not automatically the right choice for every child. Age, size, anatomy and the specific findings all matter, and Dr. Pritesh will explain which approach he recommends for your child and why.
Open Pyeloplasty
Open pyeloplasty uses a small incision in the side or tummy and remains an excellent option, particularly for young infants. Both approaches aim for the same result: a wide, freely draining junction.
CHOOSING A SURGEON
Choosing a Pediatric Pyeloplasty Surgeon
Parents searching for the best surgeon for pyeloplasty in Delhi are usually looking for someone they can trust with their child. Useful questions to ask any surgeon:
01Are they trained in pediatric surgery and pediatric urology?
02How often do they manage hydronephrosis and PUJ obstruction in children?
03Will they explain monitoring as well as surgery, not only the operation?
04Does the hospital have pediatric anaesthesia and post-operative care?

There is no single price for pyeloplasty, because the total depends on the plan made for your child. After the assessment, the clinic can share an estimate for your child’s plan and explain what it includes. Ask whether the quote covers stent removal and follow-up scans, or contact the clinic.
If you have health insurance, check coverage and pre-authorisation with the hospital before admission.
THE MAIN FACTORS
Open or laparoscopic approach
The hospital and room category chosen
Surgeon, anaesthesia and operating theatre charges
Tests before surgery, such as the renogram
Length of hospital stay
DJ stent placement and its later removal, if needed
Medicines, follow-up visits and scans
AFTER SURGERY
Recovery, Follow-Up and Risks
Recovery and Follow-Up After Pyeloplasty
Every child recovers at their own pace, and the care team guides you at each stage:
Pain relief and close monitoring in hospital
A gradual return to normal feeds and play
Simple wound care at home
A planned date for stent removal, if a stent was placed
Follow-up ultrasounds and, where needed, a renogram to confirm improved drainage
Risks and Possible Complications
Pyeloplasty is a well-established operation, but like all surgery it carries some risk. This includes:
BleedingInfectionReactions to anaesthesiaUrine leakage from the repairDiscomfort or urinary symptoms from a stentOccasionally, re-narrowing that needs a further procedure
Your child’s risks depend on their age, condition and the approach used, and Dr. Pritesh discusses them openly before you decide. No surgeon can guarantee a result, which is why follow-up continues after the operation.
Contact the clinic or seek urgent care if your child develops fever, worsening pain, repeated vomiting, reduced urine, or redness or discharge at the wound.
Has a scan shown a swollen kidney?
Book a consultation with Dr. Pritesh Maheshwari for a careful review of every scan and a clear plan, whether that means monitoring or surgery.
If a scan has shown hydronephrosis in your baby, before or after birth, an early review with a pediatric urologist tells you whether to simply monitor or plan further tests. Hydronephrosis on a report does not automatically mean surgery.

FREQUENTLY ASKED QUESTIONS
Frequently Asked Questions About Pyeloplasty
What is pyeloplasty, and why is it done in children?
+
Pyeloplasty is an operation that removes a narrowed section where the kidney joins the ureter and reconnects them so urine drains freely. In children it is done to relieve a significant PUJ obstruction and protect the kidney from further strain.
Does every child with hydronephrosis need pyeloplasty?
+
No. Many children with hydronephrosis, especially mild cases found on pregnancy scans, only need monitoring, and a good number improve on their own. Surgery is advised only when tests show a significant blockage, falling kidney function, or symptoms linked to the blockage.
How is PUJ obstruction diagnosed?
+
It is usually suspected on ultrasound and confirmed with a diuretic renogram (DTPA or EC/MAG3 scan), which shows how well each kidney works and how quickly it drains. Some children also need an MCU to rule out reflux. The tests chosen depend on your child’s age and findings.
Is pyeloplasty a major surgery?
+
It is done under general anaesthesia and needs a short hospital stay, so it is considered a significant operation. It is also a well-established, routinely performed procedure in pediatric urology. Dr. Pritesh explains the plan, the hospital stay and the recovery in detail before you decide.
What is the difference between open and laparoscopic pyeloplasty?
+
Both perform the same repair. Open pyeloplasty uses one small incision; laparoscopic pyeloplasty uses a few smaller keyhole incisions and a camera. The better choice depends on your child’s age, size and anatomy, and Dr. Pritesh will explain which he recommends and why.
Is a stent always needed after pyeloplasty?
+
Not always. Many children have a temporary internal DJ stent to protect the repair while it heals, which is later removed in a short day-care procedure. Whether a stent is used, and for how long, depends on the surgical plan for your child.
How long does recovery after pyeloplasty take?
+
Recovery varies with age and the surgical approach. Most of the early recovery happens in hospital and the first weeks at home, with follow-up scans continuing over the following months. The care team will give you a recovery plan specific to your child.
Will pyeloplasty improve my child’s kidney function?
+
The main aim is to relieve the blockage and protect the kidney’s remaining function. Published results for pyeloplasty in children are generally favourable, and in some children function improves. Outcomes depend on the kidney’s condition before surgery, and no surgeon can guarantee a result, which is why follow-up scans matter.
What affects pyeloplasty surgery cost in Delhi?
+
The main factors are the surgical approach, the hospital and room category, surgeon and anaesthesia charges, pre-operative tests, length of stay, stent placement and removal, and follow-up scans. After the assessment, the clinic can share an estimate for your child’s specific plan.
When should I see a pediatric urologist for hydronephrosis in Delhi?
+
Book a review if hydronephrosis was seen on a pregnancy or newborn scan, if a follow-up scan shows the kidney is still swollen or worse, or if your child has urine infections or recurring tummy or side pain. Seek urgent care for high fever, severe pain or repeated vomiting.
